At ACPS we know time is precious and it is only sometimes convenient to take time off of work to visit your doctor.
That is why we have pioneered a new form of consultation that allows you to have a consultation on your time, at any time from anywhere. Our online photo submission consultation is meant to help you answer those basic questions such as, “Are you a candidate for surgery”, or “How much is this going to cost” before you take time off of work.
The process works like this:
Step 1
Complete the form attached to the link below. This form allows you to submit photos, which will be important in your surgeon's ability to provide a quality consultation.
Step 2
Pay the $100 convenience fee. You will receive a phone call or email link to pay the consultation fee. This fee is applied to surgery if you choose to move forward.
Step 3
A patient care coordinator will reach out to you through email to discuss your goals, medical history, and other information the surgeon needs. She will then discuss your case with the surgeon, including showing the surgeon your photos.
Step 4
You will then receive a determination from the surgeon on your candidacy for surgery and a tentative treatment plan & quote*. Quotes will reflect a $500 discount and are valid for 90 days.
Step 5
If you are ready to move forward with surgery, the patient care coordinator will schedule an in-person consultation & consent appointment. Some patients want to take the intermediate step of just scheduling the in-person consultation. This is totally up to you.
*Disclaimers: Your surgeon will provide you with true and accurate information based on the quality of information you provide in the online form and the quality of your photos. In some rare circumstances when the information is incomplete or the photos are not representative of the actual patients the treatment plan & quote may need to be modified. The $500 discount is meant to encourage patients to try out our new online consultation process. This does not apply to any patient who already has a valid quote. Surgery must occur within 90 days from the date the quote is provided. This offer cannot be combined with any other offer.
